Question: Authentic Assessment: Informed Consent This week you will create an informed consent form that could be used with clients to notify them of their rights and responsibilities. Write a 2100 word essay addressing informed consent. Be sure to completely answer all the questions for each bullet point. Separate each section in your paper with a clear heading that allows your professor to know which bullet you are addressing in that section of your paper. Support your ideas with at least three (3) sources in your essay. Make sure to reference the citations using the APA writing style for the essay. The cover page and reference page do not count toward the minimum word amount. Review the rubric criteria for this assignment. Create a sample informed consent document that could be utilized in an advisor capacity. Begin by doing an Internet search to examine sample documents (the search term “informed consent for guidance” brings up a variety of options); you may utilize any of the samples you find as the basis for crafting your own document. In most cases, you will discover that you will find relevant components from a variety of different samples that you will want to piece together to fit your needs. As you create your informed consent, keep the following questions in mind: How would you inform your clients of their rights and responsibilities? What are some of the themes of the ethics codes with respect to the rights of clients? From an ethical perspective, why is informed consent of paramount importance? From a legal perspective, what are the three elements involved in adequate informed consent? Define these terms: capacity, comprehension of information, and voluntariness. You do NOT need to answer these questions specifically, rather you should keep them in mind as key issues as you create your informed consent.

Whether the professional is in the counseling profession or the academic advising profession, they bear a responsibility towards ensuring that the clients are entirely knowledgeable of their rights, the type of service being rendered and the reasons, the limits of privacy, and the anticipations that govern the professional relationship. Corey (2024) indicates that informed consent should never be a one-way exchange of information but a two-way exchange of information. It should be done in such a way that a friendly atmosphere is created where the clients are at ease when they want to express confusion, seek clarification, and conduct a meaningful conversation. This approach of participation enhances a sense of trust and transforms informed consent into an ongoing and not a one-time event. The principle of informed consent is based on pre-existing ethics theory and reinforced by a rich history of medical and legal tradition. It is meant to safeguard client dignity, encourage self-determination, and provide protection to their general well-being. This essay discusses the ethical and legal basis of informed consent, philosophical justifications of informed consent, and the application of informed consent in the practice of the helping professions. An informed consent document suitable for use in an advising role is also provided as a sample.
